Community Connect supports people to be fully engaged in the community. Community Connect is person-centered, whereby people’s personal interests and goals are used to direct a person’s day. Community Connect is community-based, building community partnerships to provide people the opportunity to participate in a variety of social, recreational and volunteer activities. Community Connects supports people to develop and build skills toward independence and connect people to members and services in the community.
